Carefully fill out all required fields, including:
Personal Information (Full Name, Contact Number, Active GMAIL Address)
Academic Information (Course, Subjects to Enroll)
Payment Details (Deposit Reference Number, Payment Date)
STEP 2: PAY THE REQUIRED FEES
Payment should be made via Metrobank through the following options:
Over-the-Counter Deposit at any Metrobank branch
Online Bank Transfer via Metrobank Online Banking
Gcash to Metrobank Transfer (if applicable)

Take a clear screenshot or photo of the deposit slip or online transfer confirmation.
STEP 3: UPLOAD PAYMENT PROOF TO THE GOOGLE FORM
In the Online Enrollment Form, locate the section for uploading proof of payment.
Click the "Upload File" button and attach the screenshot or photo of your payment receipt.
Ensure that the uploaded image is clear and readable before submitting.
STEP 4: WAIT FOR EMAIL CONFIRMATION
Once your form and payment proof have been submitted, the enrollment team will verify your details.
You will receive an email confirmation within [1-2 working days] with your official enrollment status.
If there are any issues (e.g., incomplete form, unclear payment proof), you will be notified via email for further instructions.
STEP 5: FINALIZE YOUR ENROLLMENT
If you receive an email stating that you are officially enrolled, check for further instructions, such as:
